The Service Coordinator plays an important role in supporting the quality, compliance, and coordination of residential services at Support Solutions. This position works under the direction of the Director of Care Management and provides guidance, training, documentation support, and compliance oversight to Residential Coordinators and residential staff.
This role helps ensure that Personal Support Plans, Individual Service Plans, Reportable Events, required documentation, and regulatory expectations are completed accurately, timely, and in alignment with agency standards. The Service Coordinator also participates in service planning meetings and supports communication between individuals served, guardians, case managers, staff, and other team members.

The Service Coordinator is responsible for supporting residential programs through service planning, documentation oversight, compliance tracking, and staff guidance.
Key duties include:
Supporting the development, review, and tracking of Personal Support Plans and Individual Service Plans
Coordinating and participating in service planning meetings with individuals, guardians, case managers, residential staff, and other team members
Writing or revising PCP/ISP goals, service descriptions, narratives, and required documentation
Maintaining tracking systems for annual documents, deadlines, required follow-up, and planning needs
Assisting with intake documentation, electronic files, Therap entry, Personal Information data, PIL/MAR setup, and required records
Reviewing and supporting Reportable Event documentation for accuracy, completeness, timely submission, and follow-up
Providing coaching, clarification, and guidance to Residential Coordinators and staff when documentation or follow-up needs improvement
Supporting compliance with MaineCare Waivers, HCBS requirements, licensing expectations, behavior regulations, agency policies, and documentation standards
Maintaining professional communication with managers, guardians, case managers, external providers, and internal team members
Participating in the agency’s on-call supervisor rotation
Other duties as assigned;

Applicants for the Service Coordinator position should be organized, professional, detail-oriented, and able to manage multiple responsibilities while maintaining strong communication and documentation standards.
Minimum and preferred qualifications include:
High school diploma or GED required
One to three years of experience in residential services, case management, human services, healthcare, compliance, or a related field preferred
Experience with person-centered planning, ISP/PCP documentation, MaineCare services, or residential programming preferred
Proficiency with Microsoft Office required
Experience with Therap, Evergreen, Saddleback, or similar documentation systems preferred
Ability to learn and accurately use electronic documentation systems
Successful completion of required agency training
Ability to maintain confidentiality and follow agency, state, and federal documentation standards
Strong written and verbal communication skills
Ability to communicate professionally with individuals served, guardians, staff, managers, case managers, and external providers
Ability to manage deadlines, maintain organized records, and follow through on assigned responsibilities
Valid Driver’s License required
Valid Auto Insurance required
Must be authorized to work in the USA

This is a full-time, exempt position within the Residential Department. The schedule and availability expectations will be discussed during the hiring process. This position also participates in the agency’s on-call supervisor rotation.Department:
